Project Details

Community Health Workers occupy a pivotal role in driving patient engagement, care coordination, and medical adherence for chronic disease such as hypertension and diabetes--key risk factors for COVID-19 that are widely prevalent in communities of color. 4-CT is working with the Connecticut Health Foundation to hire and train 20 Community Health Workers over a 6 month period and deploy them into communities of color that have been disproportionally impacted by COVID-19 as evident in both infection rates and mortality rates.
